Situation:
A fellow engineer is hanging out of the manhole. He isn’t feeling well. He’s spent 4 hours removing corrosion from the heat exchanger. It’s a summer’s day. You are the relief team.
Assignments:
- Consider possible causes for the engineer’s situation.
- Check whether adequate measures were taken for him to safely perform the task. Note any concerns on your sheet (mark as unsafe).
- Consider whether the measures indicated are adequate to prevent a repeat of the situation.
- Share any personal experiences.
